​ Referrals.

A person can be referred by their treating doctors, local hospital, treating specialists, allied health professionals, by self or by family members and friends.

The person needs to meet the following criteria in order to be eligible for the GV Hospice service:

• Lives within the City of Greater Shepparton service catchment area
• Have a diagnosed life limiting chronic or terminal illness
• Have given consent to the referral (this may include their next of kin or person responsible)
• Have care needs that are assessed as requiring the support of a specialist community palliative care (this may include the person’s family or carer/s)

GV Hospice Care is a not-for-profit home-based specialist palliative care service which aims to improve the quality of life of individuals, carers and families of those experiencing a life-limiting, chronic or terminal illness.
